Thank you for showing an interest in music lessons for your child. I would be delighted to teach them (depending on availability). I teach at Frederick Gent School and Woodbridge Junior School.

Please read the Terms of Service and privacy statement. Then complete the details on this form. I will send then you the completed contract, and an invoice for the term's lessons.

The following subsidies are available to pupils in year 4 and above. Please check if you are eligible and tick the box in the form. I will apply for the subsidy on your behalf. Only one subsidy can be claimed at a time.

• First Access Subsidy: £3.00 reduction per lesson for the first 12 lessons. The pupil must not have had instrumental lessons in school before. Pupils who have taken part in Classroom Instrumental Learning (CIL) are eligible as long as they have not also had instrumental lessons in addition to their CIL classes.

• Assisted Bursary: £3.25 reduction per lesson. Families must be in receipt of Free School Meals and/or Universal Credit. All Looked After Children are eligible for an Assisted Bursary.

• Special Education Needs and Disabilities Scheme: £5.50 reduction per lesson. Pupils must be on their school’s special needs register, and their special need must disadvantage their learning of an instrument. A letter from the school confirming the above will be required as evidence.
